There are three components you need to access in order to complete the screener:
- PDF file or online flipbook (click HERE to access the link) that contains background information and instructions (this does not need to be printed but you will need to reference it)
- Parent record form and child response form CLICK HERE
- Fillable PDF to send to your child’s school/ teacher (this will need to be downloaded and modified by entering your screener results) CLICK HERE
